    Going Home

    Your physician will decide when you are ready to be discharged from the hospital. A discharge planner may be asked to work with you to coordinate any services you might need after your hospitalization. After your discharge, you will be mailed a Patient Satisfaction Survey form. We ask that you please complete this confidential survey so that we may continually strive to improve the quality of health care that Sutter Maternity & Surgery Center provides in our community.

    If you have any questions or concerns about leaving the hospital, you may ask to meet with the discharge planning nurse at anytime by calling x2233 from your room phone. The discharge planning nurse can also be contacted prior to your hospital stay at 831-477-2233.

    Continuing Your Care

    Home Health Care
    After your hospital stay, you may need nursing care, therapy or personal assistance at home. The hospital discharge planner can help you coordinate these services.

    The Visiting Nurse Association of Santa Cruz County has been providing quality home health care services for more than 50 years. Home health care services are available 24 hours a day, seven days a week, every day of the year. You can reach the VNA by calling 831-477-2600 or visit its website.
