    When You Arrive

    Report to the admitting area located in the lobby of the hospital. An Admissions Representative will ask you some questions to confirm and update your medical record information. All information you provide will be held in strict confidence. If you are a maternity patient who has pre-registered, please go to the admitting area first before proceeding to the Perinatal Unit.

    What You May Expect Upon Admission

    • You will be asked if you have Advance Directives, which are legal documents you may prepare to direct your medical treatment, should you become unable to communicate your wishes (see section of handbook about Advance Directives).
    • You will be asked to sign a Treatment Consent and Conditions of Admission form.
    • You will receive a wristband to wear until you leave the hospital. The identification information and barcode on the band helps us ensure correct identification at all times and that you receive the medications and treatments your physician orders for you.

    Safeguarding Your Health Information

    In response to a nationwide identity protection law, Sutter Maternity & Surgery Center has implemented an identity theft prevention program to help identify, detect and respond to patterns, practices or specific activities that could indicate identity theft.

    As part of this program, hospital employees may request photo identification from you during the hospital admitting process.

    This simple step may eliminate instances of patient health insurance information and/or personal medical history being used by an unauthorized individual to gain access to medical services or medications. It also ensures we have accurate information needed to provide quality care.
